I hereby grant permission to the Yes on A for Affordable Housing committee ("Committee") the irrevocable right and permission to use, without compensation or other consideration, my visual image and likeness in photographs; my visual image, likeness, and voice as recorded on audio or video; and my name, comments and personal information in all campaign materials on the Internet and in print, audio or video produced in support of the Measure A ballot measure in the November 2016 election in Santa Clara County, California.

I understand that my image and likeness may be edited, copied, exhibited, published or distributed publically and waive the right to approve the final product wherein my image and likeness in photographs and video, voice, name, comments or personal information appear.

I hereby release the Committee and its current and former agents, directors, officers and employees from any and all claims, damages and liabilities in connection with the use or distribution of said photographs, video and audio recordings; name; comments; and personal information, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I warrant that I am eighteen years old and acknowledge, that I have completely read and fully understand the above release and agree to be bound thereby.
